  Welcome to the 2023-2024 K-12 Family Faith Formation Year at Divine Savior! The children and youth core teams are looking forward to a fabulous year of fellowship! Sessions beging on September 17th!
  
  The below enrollment form must be completed to ensure your space is held. However, your child is not officially enrolled until payment has been received. A separate email with payment information and calendars will be sent within a week of submitting the below form. Any enrollment forms received after September 21st will include a $30 service charge. If this year your child is celebrating a sacrament, a copy of your child's baptism certificate must be submitted before November 2nd to Jen Shaver at jennifer@divinesavior.com. They will not be enrolled in sac prep until the form is submitted.
